Nurse Practitioner-Full Time-
Nurse Practitioner -Health Risk Assessments
We are currently looking for BoardCertified Nurse Practitioner interested in a FULL-TIMEJOB conducting In-home Health Risk Assessments on Medicare Advantage members. Qualifiedcandidates will receive a, competitive salary-RVU Bonus, mileage reimbursement, full health benefits,401k, PTO, paid training, flexible scheduling options, autonomy & work-life balance!
Perform in-home wellness assessments withMedicare recipients to pinpoint any gaps in care. Assessments cover priormedical history, present history, and medication review. Educate members onchronic health conditions, medications, preventative screenings/tests, andlifestyle concerns.
Responsibilities of theNurse Practitioner - Health Risk Assessments
• Conduct comprehensivein-home health risk assessment to identify all active and chronic diseaseconditions as well as determine all physical, mental and social needs presentat the time of the visit.
• Obtain vital signs,measure BMI, review pharmacological therapy and conduct a physical examination.
• Educate and counselpatient and family on any conditions identified during assessment and screeningprocedures.
• Make recommendations andcommunicate findings to Primary Care Physicians through documentation ofoutcomes on provided tablet.
Responsibilities of theNurse Practitioner - Health Risk Assessments
• BSN & Master'sdegree in Nursing from an accredited school of Nursing.
• Availability to committo a full-time job.
• Board Certification withcurrent unrestricted Nurse Practitioner (NP) license.
• Experience dealing witha complex set of patients with a relatively high level of acuity.
Starting Salary: $114,000 per year + years of experience + incentives
