Job Description
SUMMARY

In collaboration with other NFP staff, provides direct patient care to newborns, infants, children, adolescents, adults, pregnant and postpartum women and older adults. This includes preventive, primary care and chronic care services in the ambulatory settings of Neighborhood Family Practice.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following description is a general representation of the key duties and responsibilities of this position. Other duties may be assigned, as required.
  • Make sound clinical judgments based on assessment of the physical, psychological, spiritual, social and environmental needs of the client.
  • Performs patient assessment, documents and analyzes data relative to patient needs, existing signs and symptoms and current patient data.
  • Utilizes critical thinking in the diagnostic process, synthesizing and analyzing the collected data and formulating a differential diagnosis based on history physical examination and diagnostic test results and in establishing priorities of care.
  • Develop a problem list, a diagnosis and patient education plan. Documents patient care outcomes and reassess and modifies the plan with the patient and family as necessary to achieve health and medical goals.
  • Recommends consultations or referrals as appropriate.
  • Accepts accountability for clinical practice including making decisions proactively, responding to changing needs of patients/families and representing patient needs to others as appropriate.
  • Prescribes drugs and therapeutic devices in accordance with Ohio laws and within the scope of practice.
  • Maintains collaborative relationship with physicians in the practice and renders care according to the guidelines of the standard care arrangement.
  • Develops, implements, and periodically evaluates therapeutic plans.
  • Orders and interprets diagnostic studies.
  • Teach and counsel individuals and families to assist them to assume responsibility for the prevention of illness and restoration and maintenance of health.
  • Communicates with community agencies to plan comprehensive care for clients.
  • Performs minor diagnostic and surgical procedures within scope of practice. Fully inform patients of risks and benefits of any procedure.
  • Exercises nursing judgment and carry out independent nursing functions responsibly and with accountability to oneself and one's patients.
  • .Exhibits cultural awareness and sensitivity in caring for the diverse NFP patient population.
  • Keeps records up to date and completes chart documentation in a timely manner.
  • Participates in annual chart review, biannual prescriptive review, regular peer review and performance improvement activities as scheduled and needed.
  • Participates as assigned in the orientation of new staff and/or serves as a preceptor to graduate APRN students.
  • Functions as a resource for other nurses, health care providers and other clinical staff.
  • Work as a team member to support NFP as a Patient Centered Medical Home (PCMH).
  • Provide on-call coverage as assigned.

QUALIFICATIONS

Specific knowledge, training or skills required to perform the duties of this position. Specific concepts, courses, training programs or required certifications:
  • Master's of Science in Nursing or Doctor of Nursing Practice.
  • Current RN and APRN Licensure in State of Ohio.
  • Board certification as Family Nurse Practitioner from ANCC or other certifying Board.
  • Meets Credentialing requirements/qualifications for clinical privileges.
  • Demonstrated understanding of the scope of nursing practice and the role of the nurse practitioner.
  • Demonstrated clinical competence, including physical assessment, health teaching and counseling.
  • Demonstrated commitment to continuing professional development and evidence-based practice through independent reading and study, and participation in continuing education programs.
  • Demonstrated understanding of public health policy and practices; familiarity with medical home model of care.
  • Demonstrated analytical and problem solving skills.
  • Demonstrated written and oral communication skills. Ability to communicate in English, Spanish preferred.
  • In-depth emotional intelligence: demonstrated self-awareness, accurate self-assessment, sensitivity and empathy, openness, reliability and consistency. Demonstrated cultural competency and understanding of underserved populations.
  • Computer proficiency: application of EMR and related functional software.
  • Demonstrated collaboration skills.

  • Demonstrated ability to sit, stand, stoop, bend and walk throughout the day. May need to sit or stand for extended periods.

Previous experience that is necessary background to qualify for this position:
  • Moderate level (3-5 years) of Family Nurse Practitioner experience.
