Responsibilities:
Great work you'll do here:As a Hospice LPN you will provide skilled and compassionate nursing care to patients and families in their home or community under the supervision of a Registered Nurse and direction of the Plan of Care. You will be responsible for visiting patients to assist in their end of life care, the documentation of care given, and chart maintenance to meet both regulatory and billing requirements.Provides services in accordance with organizational policies and procedures and regulations that define scope of practicePrepares clinical and progress notes on the relevant and specialized nursing services he/she providesAssists the RN and/or physician in performing specialized duties related to end-of-life care Assists the RN in carrying out the plan of carePrepares equipment and materials for treatment adhering to aseptic technique as required Assists the patient in learning appropriate self-care techniques Communicates changes in patient condition and needed changes to the plan of care to appropriate team members and caregiversParticipates in staff and IDT meetings as requestedEnsures the maintenance and confidentiality of client records and employee information in accordance with Confidentiality policyAttends nursing home case conferences as requestedAssists with record reviews if requested Contributes to the effectiveness of case management and coordination of services through open communicationFollows directives of case manager and supports the efforts of case coordinationCompletes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company
