Physician - Family Medicine

St. Peter's Health Partners is seeking a part-time Physician to join the team covering our Long-Term Care facilities within the Capital District, New York.

Our team provides compassionate care to seven Nursing Home facilities in the Capital District. This opportunity would be providing care primarily for the residents of Eddy Heritage House Nursing and Rehabilitation Center in Troy and Eddy Village Green in Cohoes.

Eddy Heritage House is a 120-bed facility that provides skilled nursing, sub-acute rehabilitation, and respite care to its residents. Eddy Village Green has 16 ranch-style houses, each accommodating 12 residents for a total of 192 beds. Both facilities have interdisciplinary teams to work with the providers and patients on providing the highest level of care for the residents.

Position Details:

Flexible hours; daytime position
Part-Time opportunity
Average of 12-14 resident visits per shift (consisting of routine visits and sick visits)
On call 1 in every 6-8 weeks (weeklong call; phone calls only)
Abundance of support services in the facility including providers for Pulmonary, Physiatry, Psych, WOCN, Podiatry and more

This opportunity is open to Family Medicine, Internal Medicine, or Geriatric Medicine physicians.

Requirements for this opportunity include:

Board Certified or Board Eligible
NYS Physician License and DEA
Experience with geriatric population
Certification in geriatrics a plus, but not required
Applicants should possess excellent communication skills, and a strong commitment to providing excellent care that is team oriented
