Seeking a full time RN Care Manager Supervisor to join a great team in Vermont!
- The ideal candidate will have 3-5 years care management experience and some supervisory experience mixed in there
- Position will oversee: 2 FTE RN Care Manager (1 for PCU and 1 for ED) 1 Social Work Care Manager (generally PCU) In total: 8 staff (inclusive of per diem) JOB SUMMARY The RN Care Manager Supervisor assesses appropriateness of intensity of services for patients, makingrecommendations for change, as needed. Applies utilization review criteria to daily assessments of patientsneeds. Supports medical staff in securing appropriate level of care for patients. Participates in discharge planningprocess to ensure continuity of care in the most appropriate setting. Plans, organizes and directs the CaseManagement Department including but not limited to, policies and procedures in accordance with the establishedpolicies of the hospital and in accordance with all applicable laws and regulations, scheduling of staff, preparingand reporting department budget. Coordinates with other departments to provide efficient and appropriateservices to patients and hospital staff. QUALIFICATIONS Education:o Graduate of an accredited school of registered nursing with current licensure as a Registered Nurse inthe State of Vermont. Bachelor of Science Degree in Nursing required. Master's Degree preferred. Experience: o Three years experience in a hospital setting required. Utilization management and/or case managementexperience desired. Special Skills:o Certification in utilization management expected within 2 years. Excellent verbal and writtencommunication skills required. For more details submit your resume.
