Perform health assessments and screen for preventative health
measuresActive medical management of common and complex conditionsOrder, interpret and evaluate diagnostic tests to identify and
assess patients' clinical problems and health care needsFormulate and document care plansPrescribe medication or other forms of treatment.Participate in an on-call rotation, as neededCommunicate assessment findings and collaborate in identifying
gaps in careEducation:

2+ years of medically-relevant experience preferred, however new
graduates are welcome to applyAdvanced Practice License in Connecticut (or eligibility to
apply)DEA license (or eligibility for registration)Masters Degree in Nursing Current Nurse Practitioner
certification in specialty area (ANCC/AANP)Experience with the geriatric population preferredExperience with Electronic Medical Record (EMR) systems;
eClinical Works preferredDemonstrated interest/passion for value-based care delivery
models
