Overview:
This position is responsible for assessing, coordinating, implementing and evaluating the rehabilitative care for patients with pulmonary related diagnosis. The Staff RN in Pulmonary Rehab is responsible for providing patient care utilizing the nursing process. The nurse collaborates with members of the rehab team and ancillary staff to provide effective, safe, and quality care. Functions within the framework of policies and procedures of the hospital.

Responsibilities:
1. Responsible for providing assessment along with educational classes and one on one education for patients in Pulmonary Rehab Program.1.1 Conducts appropriate assessment of patient status and rehab needs. Incorporates information into individual plan of care.1.2 Identifies pulmonary risk factors of disease and lifestyle modifications needed. 1.3 Uses age related appropriated learning strategies to meet learning needs when instructing patients, families, re: risk factor management, medication, adverse pulmonary signs and symptoms, discharge instructions and other educational needs, as documented in the plan of care. 1.4 Conducts educational classes on lung disease, pulmonary medications, breathing retraining, oxygen, pulmonary hygiene, sleep therapy, living with lung disease and smoking cessation as appropriate.1.5 Assists and communicates specific needs of patients and families when participating in pulmonary team conferences and collaborating with physicians and other team members.1.6 Keep coordinator informed of patient status.2. Conducts exercise classes in Pulmonary Rehab Program.2.1 Supervises monitoring of patients pulmonary response during sessions and evaluates patients response to therapy in accordance with patient's documented response in exercise progress forms2.2 Reports unfavorable responses to exercise therapy immediately to patient's physician, themedical director, and supervisor.2.3 Assures safety of patient during exercise sessions 100% of the time.2.4 Conducts code during medical emergency as outlined in procedure.2.5 Intervenes in complications to exercise therapy according to protocol.3. Completes required departmental records and reports.3.1 Documents the patient's response to therapy on progress forms on a daily basis (chart) and to patient's physician through verbal report and/or written progress reports on all assigned patients.3.2 Supplies statistical data to the department as needed. Compiles information for summary reports, reviews charts for accuracy and appropriate information. Insures timely delivery to medical records of completed charts. 3.3 Actively participates in maintaining inventory of departmental supplies and forms by notifying Pulmonary Rehab Supervisor..3.4 Participates in developing and implementing new forms, self-assessment tools, and educational materials as needed.4. Communicates effectively with members of the Pulmonary Rehab Department and with members of the entire interdisciplinary team including nursing and therapist.4.1 Communicates directly with physicians and other interdisciplinary team members re: changes in patient's condition, inappropriate patient response to nursing interventions, progress toward achievement of short- and long-term goals, and cancellation of therapy. 4.2 Communicates patient information/status in interdisciplinary team rounds according to established policy and procedure.4.3 Provides positive, supportive communication to patient's family and hospital personnel as demonstrated by supervisor's observations and no documented complaints.5. Exhibits leadership skills to enhance the quality of care delivered.5.1 Collaborates with pulmonary rehab clinicians in implementing interventions to provide continuity of care as evidenced by documentation of achievement of patient goals in the computerized record or chart each day.5.2 Demonstrated ability to orient new personnel as evidenced by new employee checklist being completed and new employee knowledge of rehabilitation.5.3 Demonstrates initiative in performance of pulmonary rehab care by securing and implementing physician's orders and nursing or respiratory therapist orders without direct instruction of supervisor.5.4 Demonstrates ability to establish priorities based on patient assessment of needs as evidenced by patient's progress toward achievement of established goals.6. Contributing to the management and efficient operation of the department.6.1 Maintain patient care areas in an organized manner and established policy and procedure.6.2 Labels patient supplies according to policy and procedure as evidenced by no omission of supplies being dated and labeled with patient name and room number when applicable.6.3 Maintains changes for such equipment and supplies according to policy and procedure as evidenced by maintenance of equipment log and no missing charges.6.4 Assures that equipment is properly functioning prior to and during patient use as demonstrated by no use of malfunctioning equipment with patient and no omission of completing maintenance requests.6.5 Demonstrates effective time management skills as demonstrated by completion of routine patient care assignment within shift.6.6 Follows universal precautions for all patients as evidenced by compliance at all times to facility policy for infection control.6.7 Plans for patient safety, utilizing standards as evidenced at all times with safety procedures in the safety manual.6.8 Offers assistance to other rehab employees (i.e., nurses, therapists, case managers, assistants and unit secretaries.)7. Demonstrates commitment to professional growth and competence.7.1 Attends and participated in 80% of the scheduled shift meetings when on duty and reads meeting minutes if unable to attend. 7.2 Demonstrates knowledge of current pulmonary rehab literature and practice by contributing this knowledge in clinical situations.7.3 Established and meets annual individual goals which are developed to achieve individual professional growth.7.4 TB skin tests obtained as required.7.5 Completes all mandatory modules/in-services annually.7.6 Completes competency annually within required time frame.8. Assists in delivering high quality health care services in a professional, compassionate, and courteous manner while respecting the dignity and individuality of each person who comes in contact with the organization by following the Standards of Excellence, the Standards of Behavior, and the Standards of Professional Appearance and Conduct.8.1 Maintains patient privacy and confidentiality at all times according to established procedures.8.2 Follows the Standard of Professional Appearance and Conduct and the Frazier Rehab Institute departmental dress code.8.3 Communicates effectively and courteously answering assigned telephone lines within three rings identifying self at all times.8.4 Assists visitors and patients in giving directions and information.8.5 Communicates effectively and courteously with all visitors, physicians, patients and employees.8.6 Introduces self and position to patient, families and visitors at all times.8.7 Assesses environment for safety hazards, which could harm patients, visitors, or other hospital employees, and report hazards to appropriate supervisors.8.8 Collaborates with other departments and outside agencies to meet the identified needs of the patients and the organization.9. Qualifications:
Education: Graduate of accredited RN school of nursing preferred.Required Experience: NoneLicensure: Current valid license in the State of Kentucky.Skills: Patient care, assessment and teaching, critical thinking skills, technical skills related to patient care equipment. CPR/BLS (courses provided by ProCPR or AHA), ACLS preferred. Cardiac monitoring interpretation preferred. Verbal and written communication skills.
