Responsibilities:
Essential Functions/Responsibilities:Essential functions are the duties and responsibilities that are essential to the position (not a task list). • Assists patients with tasks they are unable to do for themselves, (ADL's, feeding, grooming, mobility, etc).
• Monitors the health of patients via vital signs, glucose assessments, safety rounds, and reports as appropriate to the nurse.
• Observes patient's condition, and any changes in their physical, emotional or mental status, and communicates the status of the patient, including any changes, periodically, to a supervisor, case manager, or nurse.
• Promptly completes documentation to assure an accurate legal record of patient's care.

• Assists the team to ensure adequate stock and supply is available for patient care.
• Supports the team in keeping patient rooms and the work area safe and clean. 10%Clerical
• Assists the team to respond to patient calls in a timely fashion.
• Supports the medical team in handling paperwork.

Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.
