FMG sustains a Patient-Centered Medical Home (PCMH) care delivery model that is preferred by many practicing clinicians. Under this model, patient treatment is coordinated through a patient's primary care provider to ensure that they receive the necessary care when and where they need it, and in a manner that they can understand.

The objective of the PCMH model is to maintain a centralized setting that facilitates partnerships between individual patients and their providers and, when appropriate, the patient's family as well. Care is facilitated by registries, information technology, health information exchange, and other means to assure that patients get the indicated care in a culturally and linguistically appropriate manner and at their convenience.

      Required Qualifications:


        Graduation of an accredited college or university with a Doctor of Medicine degree. Successful completed of an approved residency
        Board certification in specialist or practice required. Must maintain state licensure and DEA certificate. Current AHA BLS certification
        Skills in providing diagnostic, preventative and or therapeutic services to patients
        Knowledge of diagnosing and treating medical problems and developing care plans
        Ability to educate patients, families, and staff in an effective manner
        Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance

        Preferred Qualifications:


          2+ years of experience
          Ability to communicate with others effectively both orally and in writing in working out solutions to problems or questions relating to the practice
          Ability to establish rapport and gain confidence in others (clinicians, patients and partners in the community)
          Ability to function as a team member

          Essential Physical Functions:


            Ability to stand and sit for long periods of time
            Ability to hear and speak verbal instructions
            Ability to read and write coherently
