Family Medical Group (FMG), now a part of the Optum and UnitedHealth Group family of businesses, is looking for a full-time Family Medicine Physician. Continue with our quick application process if you're interested in finding your life's best work.
FMG sustains a Patient-Centered Medical Home (PCMH) care delivery model that is preferred by many practicing clinicians. Under this model, patient treatment is coordinated through a patient's primary care provider to ensure that they receive the necessary care when and where they need it, and in a manner that they can understand.
The objective of the PCMH model is to maintain a centralized setting that facilitates partnerships between individual patients and their providers and, when appropriate, the patient's family as well. Care is facilitated by registries, information technology, health information exchange, and other means to assure that patients get the indicated care in a culturally and linguistically appropriate manner and at their convenience.
