An exempt position responsible for practicing continuity primary care medicine. Physicians will care for a panel of patients and provide reliable, high quality and efficient care in the context of trusting relationships. Physicians will partner with the GreenField team to provide a high level of service to patients. Current primary care areas include family medicine, adolescent medicine and internal medicine.

Primary Responsibilities:


    Conducts patient histories, physical exams, assessments and develop plans for care. Consults with colleagues when appropriate
    Develops relationships with patients
    Counsels patients on preventive health care and self-management
    Manages a panel of patients. Provides comprehensive care through direct face to face visits as well as phone calls and secure messaging
    Contributes to a positive collaborative culture and supports improvement work
    Documents care in the medical record in a complete and timely manner
    Responds to medical emergencies including use of CPR
    Shares call duty providing 24/7 for GreenField patients
    Participates in clinical infection prevention by daily cleaning of the clinic
    Maintains safe, secure and health work environments by following and enforcing standards and procedures, complying with legal regulations
    Maintains patient confidence and protects operations by keeping patient care information confidential
    Enhances health care practice reputation by taking ownership, showing flexibility by accomplishing new and different requests, exploring opportunities to add value to job performance
    Attends all team meetings and is an active participant

    Required Qualifications:


      Graduation of an accredited college or university with a Doctor of Medicine degree. Successful completed of an approved residency
      Board certification in specialist or practice required. Must maintain state licensure and DEA certificate. Current AHA BLS certification
      Ability to assess quality patient care
      Ability to work with confidential material without betraying the trust of patients, families, clinicians or staff
      Quantitative focus, creates a fact based, no blame culture
      Skills in providing diagnostic, preventative and or therapeutic services to patients
      Customer focused
      Familiar with anatomy, pharmacology, pathophysiology, clinical medicine and physical diagnosis
      Knowledge of patient assessment techniques including taking medical histories, performing physicals, evaluating health status including state of wellness, and compliance with care recommendations
      Knowledge of diagnosing and treating medical problems and developing care plans
      Knowledge of documentation in medical records in confidential manner
      Ability to make responsible decisions with scope of their specialty
      Ability to collaborate effectively with colleagues and specialists
      Ability to educate patients, families, and staff in an effective manner
      Ability to demonstrate eye-hand coordination, full range of motion, and manual dexterity
      Ability to use MS Office and electronic medical record efficiently
      Skill in gathering and analyzing physiological, socioeconomic and emotional patient data
      Skill in accurately evaluating and sometimes patient problems in person or via phone or secure messaging and providing appropriate advice, intervention, or referral
      Skill in developing/revising patient care plan based on patient status
      Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment to ensure compliance

      Preferred Qualifications:


        2+ years of experience
        Ability to communicate with others effectively both orally and in writing in working out solutions to problems or questions relating to the practice
        Ability to establish rapport and gain confidence in others (clinicians, patients and partners in the community)
        Ability to function as a team member

        Essential Physical Functions:


          Ability to stand and sit for long periods of time
          Ability to hear and speak verbal instructions
          Ability to read and write coherently
