Serenity Hospice Care - *****REGISTERED NURSE - RN - CASE MANAGER - HOSPICE - Essex/Union Counties: Serenity Hospice Care is looking to hire a 'Registered Nurse (RN) Case Manager Team Lead to join our team of professionals for Essex/Union/Bergen/Passaic/Morris Counties.
Registered Nurse RN Job duties can include, but are not limited to:
Orienting new RN's.
Assisting with RN questions and needs.
Preparing for bi weekly team meetings
Running Bi Weekly Team Meetings
Ensuring that best practices are always being adhered to.
Scheduling orientations for new RN's.
Completing Competencies and Performance Evaluations
Meeting with employees for review and putting necessary plans of correction into effect.
Reviewing all RN documnetation
Reviewing all CHHA Care Plans
Counsels the patient and family in meeting end of life needs
Initiates appropriate preventative nursing procedures
Registered Nurse Supervises the Hospice Aides and LPN's
Must be very knowledgable with Medicare Regulations
Must have a firm understanding of appropriate Hospice Diagnosis
Effective communication and relationship building skills
The proper candidate must have a current New Jersey RN Registered Nurse License in good standing, must be familiar with and willing to travel the area and have a minimum of 2 years hospice experience. Must also have reliable transportation and insurance. Monday through Friday 8:30am to 4:30pm. Care for Hospice patients in their homes and facilities.
