Summary:
To coordinate all discharge needs including home going teaching, equipment and nursing needs for patients referred to CHCG. Begin admission process and coordinate information needed by other staff members.

Responsibilities:
1.Receives referral information and schedules assessments/teaching.

2.Communicates with unit discharge planners and other staff members regarding potential referrals.

3.Able to asses patient needs, implement plan of treatment, develop plan of care, arrange interventions as identified, and evaluate effectiveness of the interventions.

4.Identify priorities of patient care and ensures that patient identified needs are met.

5.Makes appropriate referrals according to identified patient needs.

6.Instructs patient/family members in all aspects of care as appropriate.

7.Documents completely in accordance with the Policy and Procedure of the Home Care agency and submits to billing.

8.Obtains information on home going needs and completes all proper documentation

9.Obtains accurate orders from the Physician.

10.Instructs patient and or family in techniques needed for home care or assigns appropriate staff to teach the family.

11.Arranges for alternative provider if needed because of location or preferred provider status.

12.Notifies the scheduler and or nurse of time and date of discharge to facilitate scheduling.

13.Case manages all short term patients and obtains any authorizations needed.

14.Facilitates customer service issues with families.

15.Reviews and resolves payor issues.

16.Participates in performance improvement initiatives such as chart reviews and other areas needed.

17.Other duties as required.

Other information:
Technical Expertise
1.Experience in home care preferred.

2.Demonstrates sound and proficient nursing skills

3.Experience working with all levels within an organization is preferred.

4.Experience in healthcare is preferred.

5.Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.

Education and Experience
1.Education: Graduate from an accredited School of Nursing. BSN preferred or must obtain BSN within 5 years from date of hire into role.

2.Certification: Licensed registered nurse in the State of Ohio. BLS required.

3.Years of experience: Minimum 1 year experience in home care or pediatrics.

4.Years of experience supervising: None
