Our Community Care division is currently seeking a well qualified Registered Nurse to join its team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following; however other duties may be assigned:
Conducts on-site assessments and develops service plan in conjunction and with the authorization from DHS.
Assess adequacy of member service plan and the need for other services on an ongoing basis, making appropriate referrals.
Provides nursing services, treatments, preventive and rehabilitative procedures requiring specialized skills under the direction of the attending practitioner.
Teach, supervise and counsel the member and family members regarding the nursing care needs and other related problems of the member in the home.
Observe signs and symptoms and report to the practitioner reactions to treatments, including drugs and changes in the members physical and/or emotional condition.
Develop and implement nursing care plans, including revisions of the plans when necessary.
Communicates to the Director and/or office personnel pertinent member and/or attendant information.
Record practitioners orders and make them a permanent part of the member record.
Completes visits as scheduled and reports verbally to unit regarding completion of visits on a daily basis.
Recruits, hires, trains and supervises attendants.
Completes appropriate forms and attaches them to Daily Visit Reports and submits within 48 hours of visit.
Corrects and resubmits error rejections within 48 hours.
Responsible for maintaining nurse files.
Maintains confidentiality of member, attendant and staff information.
Establishes rapport with DHS by making visits to case workers office in assigned area.
Participates in staff meeting and in-services.
Participates in community relations activities.
Work with other community agencies to coordinate services to meet member needs.
Participate in case conferences with other members of the health care team.
Will assume on-call duties as assigned.
Responsible for information in the Provider Manual and up-dates.

Qualifications:
Graduate from a National League for Nursing accredited school of nursing.
Competent to carry out any skilled procedures that the practitioner might order.
Previous public health or home health nursing experience is preferred, but not required.
Currently licensed to practice professional nursing.
Must have interest, understanding and appreciation of the problems of the elderly and disabled.
Must be able to make independent home visits performing all necessary tasks.
Must have appropriate nursing skills and judgment.
Must be able to handle angry or hostile individuals or situations in a professional manner.
Bilingual skills preferred (Spanish).
