Practices professional nursing by the performance of both independent nursing functions and delegated medical functions in accordance with accepted practice standards. Such functions include the initiation and performance of nursing care through health promotion, supportive or restorative care, disease prevention, evaluation/management, and diagnosis/treatment of human disease, ailment, pain, injury, deformity, and physical or mental condition using specialized knowledge, judgment, and skill involving the application of biological, physical, social, and behavioral science principles. CRS 12-38-103 (10). Will provide virtual care services to members of all ages across service area, including chat, video, telephone and inbasket.

Essential Responsibilities:


    Serve as contact for KPCO (Kaiser Permanente Colorado) patients seeking virtual care:
    Conducts telephonic and virtual encounters as appropriate within target timeframe
    Assesses patients virtually and escalates care in response to potentially urgent/emergent situations without the benefit of face-to-face interaction.
    Remains engaged with patients throughout their virtual time.
    Obtains a pertinent medical history, reviews relevant laboratory or radiologic (or other) studies to assess the patients medical, physical, psychological, and relevant socioeconomic status.
    Formulates an accurate and appropriate diagnosis based on elicited data, and includes differential diagnoses as appropriate
    Develops and implements treatment plans that are aligned with the practice preferences of the collaborating physician(s) including orders for medication and/or other treatment modalities, the need for additional testing, consultation to other providers, and follow-up provisions.
    Provides care with attention to safety, efficiency, and cost effectiveness.
    Plans are appropriately prioritized and adapted to the patient and circumstance.
    Integrates care services and proactively addresses members needs in alignment with regional and national strategies.
    Educates and counsel patients and their families regarding their treatment plan, including disease prevention and health promotion.
    Promotes the availability of culturally sensitive health and resource information that is evidence-based, patient centered, and enables informed choice.
    Documents visit, plan of care, diagnosis, and patient instructions in the medical record, including name(s) of any consulted physicians or other providers. Uses standard smart phrases, preferred documentation template for each venue, escalation pathways, and communication standards.
    Takes necessary and appropriate actions in emergency situations.
    Performs other clinical/administrative duties as directed.

    Build relationships and rapport through empathetic language.
    Able to consistently reference reviewing chart/ history.
    Demonstrate understanding of patient status and propose a plan to meet patient needs in a medically appropriate manner.
    Confident and comfortable with making recommendations toward resolution through virtual care channels, often with limited information.
    Lead patients to definitive actions.
    Know their personal limitations and when to seek advice and/or consultation from other members of the health care team.
    Understands and acknowledges the impact of personal values and cultural differences on the patient-provider relationship.
    Understands and successfully navigates the unique aspects of virtual care delivery and motivated to promote virtual care as a clinical resource for patients.
    Professional Expectations:
    Assumes responsibility for maintaining clinical competence and proficiency through participation in continuing education and other activities.
    Maintains professional certification and active licensure.
    Participates in the training and/or mentoring of new employees, colleagues, and students.
    Participates in quality assurance activities including the Peer Review process.
    Has the quality of their care evaluated by the Collaborating Physician(s) as stipulated by State and/or KFHP/CPMG policies.
    Evaluates personal performance and establishes annual goals aligned with KFHP regional expectations.
    Maintains a working knowledge of, and complies with relevant (Federal, State, and/or Organizational) laws, rules, and policies, including licensure and certification.
    Educates and promotes the roles of the APN/PA to patients and members of the health care team.

    Basic Qualifications:

    Experience


      Minimum of three (3) years of clinical Nurse Practitioner practice treating all ages in an Urgent Care, Acute Care Hospital, Emergency Department or Family medicine setting. (New Graduates not considered).

      Education


        Graduate degree in nursing Graduation from an accredited graduate nursing program approved by the Colorado State Board of Nursing.

          APN Prescriptive Authority Certification (Colorado)


            Advanced Practice Nurse Registration (Colorado)


              Registered Nurse License (Colorado)


                National Provider Identifier


                  Drug Enforcement Administration Registration


                    Nurse Practitioner National Certificate

                    Additional Requirements:


                      Must obtain and remain credentialed by the Colorado Permanente Medical Group by the start date.
                      Education and Certification must be congruent with population focus of the position.
                      Taxonomy codes are required at time of hire.
                      Must have full prescriptive authority.
                      Well versed in or willing to learn resources specific to KPs integrated care delivery system (including primary care, urgent care and other virtual or on demand care delivery options) to ensure members receive care in the most appropriate setting.
                      Strong foundational knowledge of virtual applications and software.
                      Willing and able to license in other states if requested. Subject to legal, compliance and practice considerations.
