NURSE CASE MANAGER. Carolina Case Management, based out of Raleigh, is seeking experienced and outstanding nurse case managers in the N.C. Triad area. RN or CRC or CDMS is required. Two years of work in workers compensation is highly preferred. CCM is a plus. Bilingual is a plus.
Job Requirements:
Provide medical management to injured workers to assist them in reaching a pre-injury/pre-illness level of functioning to returning them to a productive lifestyle. Activities include, but are not limited to, coordination of medical care and services, exploration, counseling, assessment of physical and psychological factors, cost analysis and negotiation, community resources referrals, development of Rehabilitation Plan, implementation of plan, and appropriate follow-up.
