Cedarwood Plaza, a 115 bed skilled nursing facility located in the heart of Cleveland Heights' Cedar-Fairmount district and a short drive from University Circle, and downtown Cleveland, offers a warm and comfortable environment with staff committed to treating everyone like family- with compassion, respect and kindness. The staff earned the Bronze National Quality Award from the American Health Care Association in 2020. The facility offers a dedicated rehabilitation unit with private suites for short-term patients, along with long-term care, skilled nursing, hospice care, and respite care.

Job Summary We are looking for dependable, professional, and detail-oriented Licensed Practical Nurses ( LPN s ) to join our highly respected team of professionals. LPN s will act as compassionate, knowledgeable charge nurses and will provide residents with highest quality care to meet their clinical needs.

Job Responsibilities Candidates will be responsible to:
• LPNs will provide direct care in accordance with facility policies & procedures and standards of clinical care
• LPNs possess a strong understanding of long-term care federal and state regulations
• LPNs collaborate with physicians, registered nurses and other healthcare professionals to provide the highest quality care to all residents
• LPNs Administer and accurately document prescribed medications and treatments ordered by the medical staff
• LPNs evaluate for assessment, identify, and document resident status and condition changes in a timely and compliant manner
• LPNs provide patient education and teaching as needed
• LPNs work independently and lead state tested nursing assistants (STNAs) in providing quality care to facility residents based on person-centered care plans
• Other duties as required

Education and Experience Requirements
• Graduate from an accredited nursing school with an active LPN license required
• Excellent organization and communication skills required
• Ability to multi-task and remain calm under pressure required
• Excellent customer service skills required
• CPR Certification Required
• Basic computer skills required; experience with Point Click Care preferred
