LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients across the care continuum focusing specifically on Skilled Nursing and Long-Term Care facility settings, to improve the quality of care delivered, refine facility processes, and enhance patient relationships. LTC provides Primary Care Physician (PCP) services and Medical Directorships to over 30 SNF-LTC facilities across Michigan.

LTC is seeking dedicated Physicians, Nurse Practitioners and Physician Assistants to join our team to serve as medical directors and primary care providers in the family of skilled nursing facilities we service.

Shift Structure
  • Flexible Schedule
  • Providers will go directly to their facilities from their home.
  • Bi-Weekly Care Meetings at LTC offices or via MS Teams
  • Rounding times are flexible with no set start and end time. Preferably, physicians start in the morning between 7-10am and finish their day when their caseload has been seen.
  • No nights or weekends
  • Normal on-call rotations
  • This is an in-person position

Requirements and Qualifications
  • Board Certification in Family Medicine, Internal Medicine, or Geriatrics
  • Candidate must hold a valid and unrestricted medical license in the state of Michigan.
  • Candidate must hold a valid DEA license in the state of Michigan.
  • Candidate must have completed a valid U.S. residency program
  • Candidate must have completed a medical degree (MD or DO) from an accredited institution

Compensation and Benefits
  • Competitive compensation package:
    • Compensation ranges from $180,000-$225,000 annually
    • RVU - Quality Bonuses - no cap
    • Monthly Medical Director Stipends (as applicable)
    • Mileage reimbursement
  • $1,500 Annual CME allowance
  • All Medical licensure expenses reimbursed by LTC
  • Malpractice insurance w/tail
  • 401k with employer matching and participation
  • Medical, vision, and dental insurance
