Position Summary:
The Oncology Nurse Navigator, under general administrative direction will act as a liaison and coordinate Oncology patient referrals from Physicians, coordinate the Oncology patient's plan of care with an interdisciplinary team, work with Physicians to develop and maintain clinical policies and procedures, establish and monitor performance improvement measures for the Oncology program, provide a significant level of patient and family education related to their illness and treatment plan, serve as a point of contact for the Oncology patient, provide on-going education to nursing on the care of the Oncology patient, work with community partners and resources to provide continuum of care as patients need, and perform within all other job duties as assigned.

Essential Duties:

1. Oncology Program development and maintenance of requirements to meet the Commission on Cancer Accreditation.
2. Work closely with Physicians of all specialty areas to coordinate the Oncology patient's plan of care and communication of that plan of care with the interdisciplinary healthcare team.
3. Policy, procedure and protocol development for the Oncology program.
4. Establish performance improvement measures for the Oncology program using PDSA methodology to ensure the provision of quality care.
5. Provide education to patients, family members, the community, and hospital associates.
6. Work with community partners and National Cancer Resources to provide support and continuity of care for the Oncology patient.
7. Performs all other responsibilities as assigned
Pay range: $51.18 - $81.91

Individual wages are determined based upon a number of factors including, but not limited to, an individual's qualifications and experience
