Job Description
Faith-based family health care center seeking a full-time (40 hours per week) Behavioral Health Nurse Practitioner to join our growing team of providers! The Behavioral Health Nurse Practitioner will care for patients who are diagnosed with behavioral health issues. Provide services for patients including assessment, diagnosis and treatment of behavioral health conditions. Based on clinical evaluation, develop treatment plans designed to provide effective management of patient care. Serve as a resource for staff, patients and families. Work as an interdisciplinary team member with other health center staff to coordinate and optimize the Behavioral Health program.
This position offers a $5,000 bonus upon completion of the first year of tenure and a $5,000 bonus upon completion of the second year of tenure.

QUALIFICATIONS
- Graduate of an accredited school of nursing
- Graduate of an accredited Nurse Practitioner Program
- Current Advanced Practice Registered Nurse (APRN) Licensure
- Three years of experience working with patients identified as having behavioral health issues
- Bilingual/Spanish speaking a plus
KEY JOB RESPONSIBILITIES
- Conduct behavioral health evaluations and assessments
- Develop treatment plans designed to optimize patient's behavioral health.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Provide individual or family therapy for patients with acute or chronic mental health disorders.
- Monitors the effectiveness of interventions
- Work independently and with other members of the treatment team
- Document all relevant patient in the medical record in a timely manner
- Formulate, execute and review healthcare education delivered to patients and families for disease prevention
