Responsibilities
Reporting to the Nurse Manager, the RN Case Manager is a registered professional nurse who is responsible for providing safe, appropriate nursing care to patients in their place of residence. Is responsible in providing intermittent professional nursing services, and teaches and supervises patients and caregivers. These services are performed in accordance with physician?s orders.
Qualifications
MINIMUM ESSENTIAL EXPERIENCE Minimum (1) year prior professional nursing experience required. Hospice and/or home health experience preferred.
REQUIRED LICENSURE / CERTIFICATIONS Current California license as a Registered Nurse required. Current BLS (CPR) required.
Current California driver's license and vehicle insurance required.
