Position Description

The Palliative Care Nurse Practitioner provides palliative care treatment and consultation to chronically ill patients under the supervision of one or more Physicians to optimize quality of life. The nurse practitioner is an extension of the Physician being able to provide full palliative care across all patient types. The Nurse Practitioner is able to prescribe therapy or medication, order and interpret tests and develop comprehensive treatment plans in cooperation with the patient's primary care provider.

Essential Duties and Responsibilities
  • Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care
  • Provides comprehensive clinical and psychosocial evaluations of patients and their families in relation to their quality of life goals
  • Serves as patient advocate in development of patient centered plans of care
  • Collaborates with patients and families to establish goals of treatment and advanced directive options
  • Collaborates with primary care to ensure interdisciplinary communication as well as treatment goals
  • Open and responsive to diverse backgrounds and experience of other people and promotes an environment that is sensitive to cultural diversity
  • Understands and is able to discern continuum support services as adjunct assistance for patient stability
  • Acts as an advocate for USMM continuum of care in the community
  • Demonstrates knowledge of Medicare benefits, patient rights, advance directives, and community resources
  • Demonstrates proficiency in concurrent EMR documentation and appropriate coding

REQUIRED Knowledge, Skills, and Experience
  • Advanced Practice Nurse with appropriate active and unrestricted state licensure
  • Valid NPI#, DEA and CTP
  • Enrolled with Medicare or state Medicaid or eligible for enrollment
  • 3 years of home based or hospice care or relevant experience
  • Demonstrates excellent observation, verbal and written communication, problem solving, and organizational skills
