Physician (Primary Care)

The Cincinnati, OH VA Medical Center is looking for well-qualified Physicians to fill full time Physician positions located within the Primary Care Department.

Work Schedule: Monday-Friday, 8 a.m. - 4:30 p.m.

Physician position located within the Home Base Primary Care (HBPC) section of Primary Care - This Physician position is located in Home Base Primary Care (HBPC). This physician will be covering the Hamilton Ohio Community Based Outpatient Clinic (CBOC) and the Dearborn County Indiana CBOC with potential cross-coverage for the Clermont, Georgetown, Bellevue and Florence CBOCs. This position is necessary to sustain open access for new and established patients at the 6 Home Based Primary Care locations. Home Based Primary Care (HBPC) is a group effort between the Veteran, family, caregiver and VA. The program uses a multi-discipline team of health care professionals (nurses, doctors, social workers, dietitians, etc.) who work together to create a plan of care specific to each Veteran. This position serves as a staff physician; Collaborates with the HBPC Program Director and Medical Director; Collaborates with HBPC PACT personnel in the ongoing management of patients; Plans and directs the educational and clinical experience of medical students, residents and fellows assigned to the HBPC program; Provides comprehensive care for veteran population such as diagnosis, care, and treatment of assigned patients; Performs assessments, diagnosis, treatment and outcome measures of healthcare problems; Provides ongoing medical care to patients with acute and chronic health problems; Provides preventative medical care including screening and vaccinations. This physician assumes a leadership role in the development and implementation of HBPCs process for continuous performance improvement; Collaborates with the HBPC Program Director and Medical Director in developing plans/strategies to meet the guidelines of the HBPC PACT directive for admitting high risk/high needs Veterans; Consults on the appropriateness of accepting Veterans consulted to HBPC; Refers to specialists when additional input is desired and required for symptom and disease management; Participates in HBPC Multidisciplinary Pain Initiative; Responds to patient inquiries that arise from daily practice, through the Patient Advocate, through the Front Office, and by Congressional action; Routine home visits to evaluate the patient; Orders test, x-rays, EKGs, and other diagnostic procedures needed to promote care; Consults with veterans and their families by telephone, when necessary to ensure quality comprehensive care is available to the veteran enrolled in the program; and Maintains appropriate and timely documentation in the medical record of clinical services performed to include the completion of clinical reminders and alerts. Additional clinical duties as delegated.
Physician (Float) position in Primary Care - This physician will provide care to patients at any of the following locations where coverage is needed: the main hospital in Cincinnati OH, Dearborn County Indiana CBOC, Bellevue KY CBOC, Florence KY CBOC, Clermont County Ohio CBOC, Hamilton, OH CBOC, and Georgetown OH CBOC. This Physician will provide interim coverage for primary care physician vacancies and for physicians on extended leave. This position is necessary to sustain open access for new and established patients. This provider will be available to provide care to our veterans in the event that a regular provider is unavailable due to illness or leave. This physician will provide primary care to a panel of patients as a member of a team including an RN, LPN, and clerical assistant; Provide care to patients with acute and chronic health problems; Provide preventative medical care including screening and vaccinations; Assist personnel on other primary care teams when the team provider is absent; Partner with community physicians, inpatient physicians, and the emergency department to provide coordination and continuity of care; Work as a team with services including pharmacy, social work, mental health, and nutrition to provide comprehensive care; Refer patients to specialists when additional input is desired and required for symptom and disease management; Refer to specialists when additional input is desired and required for symptom and disease management; Collaborate with mental health and specialty personnel in the ongoing management of mutual patients; and Participate in administrative duties including peer review, hospital committees, performance improvement, and other duties as assigned.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement.

Basic Requirements:

United States Citizenship.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.). Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), OR (2) Those approved by the American Osteopathic Association (AOA), OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Proficiency in spoken and written English.
A current, valid Drivers License.

Preferred Qualifications:

Board Eligible / Board Certified in Internal Medicine or Family Medicine is desired
Experience in geriatrics and longitudinal patient care is desired

Physical Requirements: Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. Heavy lifting, 45 pounds and over; light carrying, under 15 pounds; reaching above shoulder; use of fingers; both hands required; walking (8 hours); standing (8 hours); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); mental and emotional stability; clear speech. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Rigid deadlines and need for high degree of accuracy periodically becomes stressful.
